Spay/Neuter Agreement. Exhibit A Permanent ownership of this dog is contingent upon your compliance with this Addendum. Proof of the spay/neuter has to be received within four (4) days after the surgery. In the event that the dog’s health does not allow this agreement to be honored, CHAIN OF LOVE ABANDONED ANIMALS RESCUE, INC. must be provided with a statement from your veterinarian that this dog is not yet in physical or emotional condition for surgery. Based upon the veterinarian’s evaluation of this dog the ADOPTER will contact the veterinarian and establish the earliest date that the procedure can be performed. ADOPTER will notify CHAIN OF LOVE ABANDONED ANIMALS RESCUE, INC. of this amendment in writing, at which time all other conditions of this agreement will apply and be enforced. This is the responsibility of the ADOPTER, not the veterinarian, to ensure that CHAIN OF LOVE ABANDONED ANIMALS RESCUE, INC. has received verification that the surgery has been performed. Failure to comply with this agreement by the date below, unless otherwise agreed to in writing; will be considered a breach of the Contract, and ADOPTER will transport this dog back to CHAIN OF LOVE ABANDONED ANIMALS RESCUE, INC. and will not be entitled to a refund. I agree that this spay/neuter agreement constitutes final notice of the spay/neuter requirement and no further notice is required to institute legal action for violation of the terms of this contract. All fees (legal and investigative) accumulated from enforcing this contract will be the sole obligation of the ADOPTER. I have read and understand the above and agree to abide by its contents: By my signature below, I agree to have this dog spayed/neutered by no later than , and I understand that this spay/neuter agreement is an agreement that this dog will not produce a litter of puppies either as purposely bred or by accidental breeding. I also agree that should puppy not be altered that I will return said puppy back to CHAIN OF LOVE ABANDONED ANIMALS RESCUE, INC. with no refund.

Spay/Neuter Agreement. All The Doodles sell pet home only puppies with no breeding rights unless you are accepted into our breeder partnership program. Buyer agrees to have the dog spayed/neutered by 6 months of age or a fee of 3 times the cost of the dog ($9000) will be imposed. Please see attached spay/neuter contract.
Spay/Neuter Agreement. CAT/KITTEN IS PURCHASED FOR PET. Xxxxx agrees to spay or neuter this cat/kitten within 1 year of its BIRTHDATE. If not altered within this time frame, Xxxxx agrees to pay the additional price of Two Thousand Canadian Dollars ($2,000.00 CAD), as this would constitute the sale price of a breeding animal. If Buyer fails to provide either, Flying Folds Cattery has right to reclaim cat. Buyer must send to Flying Folds Cattery written CERTIFICATION OF ALTERATION from a licensed veterinarian. If Buyer reports to Flying Folds Cattery that this cat/kitten has died before being altered, Xxxxx must provide Flying Folds Cattery with written verification of death signed by licensed veterinarian or be liable for the additional price.
